You need to complete 2 Waivers. One is an online form, and one is a paper form that will need to be physically turned into Joel.

      1. Use the group name “Iowa 2026”

      2. Use the trip start date of 5/28/2026

      3. Each adult and minor traveling without a parent will fill out their own waiver

        1. minors who are traveling with parents will be added to ONE of their parent’s forms

  2. Madison Church’s liability waiver.

    1. This MUST be printed out 2-sided, I can not accept it on two different pages. you can get the form from me at the Church upon request, or you can print it out yourself here: Madison Liability Waiver.
